You are entitled to choose any participating dentist on the OptumHealth Allies Dental Program. Dental offices are located throughout the United States. The dental provider listings on DentalPlans.com are provided directly from each dental network or discount dental plan, which are responsible for its accuracy and completeness. Every effort has been made to ensure that only participating active providers are listed. We strongly recommend that you confirm that a provider is currently participating before you receive any services from that provider. Providers are subject to change without notice. Not all specialties may be available in all areas. No referrals are required. Fees may vary by region. OptumHealth Allies is not the provider of services under this plan. While OptumHealth Allies has made every effort to ensure the highest quality in our network providers, it is your choice as a patient to determine the nature of service you receive. For this reason, OptumHealth Allies cannot and does not endorse or recommend the services of any providers in the network. OptumHealth Allies makes no representations or warranties about the type of service, quality of care, source of payment or the billing practices of any providers. A member's participation in this plan is governed by the terms of the membership agreement provided upon joining. Please see Member's Agreement for more details.

The OptumHealth Allies Dental and Vision Discount Program is administered by HealthAllies, Inc., a discount medical plan organization located at 505 N. Brand Blvd., Suite 850, Glendale, CA, 91203. OptumHealth Allies is not insurance. OptumHealth Allies provides discounts at certain health care providers for medical services. OptumHealth Allies does not make payments directly to the providers of medical services. The program member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount plan organization.

You may cancel at any time. If you cancel within 30 days of enrollment (within 30 days of receipt of membership materials for residents of CO, IN, MO, MT, ND, OH, OK, SC, and SD), you will receive a full refund. (The application fee, if applicable, is not refundable except in AR, CO, MD, and TN.) The program and its administrators have no liability for providing or guaranteeing service or the quality of service rendered.
