How does a patient join a discount dental plan?

Patients can join a discount dental plan online at www.DentalPlans.com or by calling our toll-free number, 1-888-632-5353. Remind your patient to use your dental practice’s unique coupon code to receive a discount on their plan and track the sale to your office.

Why do we need a coupon code?

Your coupon code will give your patients a discount on the price of the plan, and it tracks the sale to your office for reporting of monthly commissions.

When are commissions paid out?

Commissions are paid out once a month, usually around the 10th. ACH or Direct Deposit is the quickest, easiest, and fastest way for you to receive your commissions. Click here to download our Electronic Payment Authorization form.

What is ACH?
ACH (Automated Clearing House) is a secure network used by Banks for direct deposits and electronic payments. ACH allows DentalPlans.com to pay your commissions electronically directly to your bank account, speeding up the time it takes for you to receive commission payments, eliminating extra postage, and conserving our environment by going "paperless."

Why is a W-9 form required?

We are required by IRS Tax Code law to obtain a tax identification number when making a reportable payment to someone who is not our employee. Failure to provide this information could result in our being required to withhold up to 30% for federal tax withholding and penalties.

Click here to download a W-9 form. Please complete a W-9 form and fax it to 954-923-2694 (Attention: Tracy Kendall) or mail it to the address below. If your earnings from DentalPlans.com exceed $600 in any given year, you will receive a 1099 form from us prior to January 31st.

Are there additional creative materials we can obtain from DentalPlans.com?

Yes. DentalPlans.com has an internal creative department that can create many different, effective marketing materials for print, web, and other media to help generate business for your dental practice. If your dental practice has a website, DentalPlans.com has banners, text links and website templates available. Our banners come in all shapes and sizes, and are available for immediate use. We will also create customized creative materials for your marketing campaigns. You can find all our creatives by logging into your Provider Affiliate account and going to the Online Creatives section.
