Simply present your membership card to the vision provider you choose
from the Plan's wide network. Choose the services or merchandise that
best suit your needs. Signature Wellness Plan members have no restrictions on
quantity or selection.
Once services have been rendered, you will be charged according to the
Signature Wellness Plan Vision Fee Schedule. While your discount is immediate,
you are responsible for paying the provider at the time of service for
all merchandise and services. Your membership kit will provide the fee
schedule for different vision services.
Vision discounts mentioned on this Web site are based
on 2001 national average charges for vision services. Some vision centers
may require that eye examinations for contact lenses be performed at their
location.

To start getting your great discounts for chiropractic services, all you
need to do is show your Plan membership card at a participating chiropractic
center. You'll receive your discount on exams, treatments, and x-rays. Your
savings will automatically be deducted from your bill.
Chiropractic patients can expect the provider to take a medical history and
conduct physical, neurological, and orthopedic examinations. Chiropractic
physicians assess posture, the spine, reflexes, joint motion, and muscle
strength. X-rays and other diagnostic images are often used to locate
vertebral dysfunction and rule out disorders, such as tumors or fractures,
which require the attention of other health care experts.
Chiropractors may order laboratory tests, give nutritional advice, and use a
variety of treatment options, including ultrasound and electric and heat
therapy.
Chiropractors do not perform invasive procedures, such as injections,
transfusions, or surgery, nor do they prescribe medication.

The Beltone hearing care practitioner will guide you through the steps of a
thorough hearing evaluation. These include:
• A complete case history, including medical questions, questions about your
hearing, and any previous use of hearing aids
• A visual examination of your ear using an otoscope
• Tests of your ability to respond to a variety of sounds
• Tests of your ability to recognize and repeat spoken words
• In some cases, a test of the mobility of your eardrum and of hearing reflexes
After your evaluation is complete, your local Beltone representative may
recommend various styles and sizes of hearing aids. Once you have been fitted
for your hearing aid, you will receive a 15% discount. The discount is instantly
applied and you will be required to pay for the hearing aid upon its receipt.
